I will add something to what I wrote.

In photo #7, note the position/alignment of the right forearm/elbow to the plane and clubshaft.

I realize this may not be the very top, but with all that is being tossed about, I am not sure that most of those comments are not just symptoms.

7-3 discusses the position for both hitters and swingers of the right forearm. Granted it appears that your left wrist maybe arched, which can take the secondary lever off plane, but your alignment of the right forearm appears to be caught between hitting and swinging. It should either be vertical to the plane or vertical to the secondary lever you are kind of inbetween.

Given this alignment, if the photos and analysis are correct could in fact address the symptoms and descriptions noted in the down stroke and follow through.

Plus I can't tell if you upper body has any horizontal sway, but you definitely have some vertical motion with the upper body, which most likelly results in a lack of a stationary center.
