Does anyone have an idea of what this might be? My thinking is that my right hand may get to on top of the club, possibly altering the position of my right forearm, but this is just a guess. Any help very much appreciated.

I remember G.S.E.D., George Kelnhoffer saying something like this concerning heel hits, "too OUTSIDE IN, or INSIDE OUT... it is the OUT part that is the problem."
With Toe or Heel hits, the sweetspot is off plane.
