In addition to the setup above, you need more 'up'. Likely you are rolling the club way inside and very flat. For full swing shanks, it can be helpful to practice in slow motion with your back to a wall (stand about a foot away at most). Also, find a low bench, table, or on the course you can use 'roped off' areas if they are strong enough - and swing back and through, back and through with the shaft resting on the bench/table/rope.
